- Print and complete Media Release Form.
- Include a copy of your child's Baptism Certificate.
- Turn in forms with payment in one of the following ways:
- Bring forms with payment to Immaculate Conception Faith Formation Office,
- email completed forms to [email protected]
- Mail all forms with payment to:

Registration Fees for Grades K - 10:
1 - 2 children………>……$40.00 per child
3 or more children….... $100.00
*2nd graders add $15.00 to registration fee (Covers retreats, certificates)
(Out of Parish or Not Registered w/ICC) fee......$25 per family
Confirmation (11th grade) Fee:
Each child.............................$70.00 (includes retreat, registration, certificate, etc.)
